Services provided to Home Care Agencies

In home assessment of needs review and report:

  • Mobility tips, barriers to proper nutrition, home assistance devices, modifications to home to increase independence 
  • Caregiver training
  • Medication: set ups and overview, or non skilled staff education
  • Provide staff training and education on mobility tips, home aids and simple modifications
  • Understanding dementia related changes with perception and mobility
  • Diabetes education and training for non skilled persons

***By not having a nurse on staff as an employee, but the ability to have a nurse per diem which you don’t need to provide benefits or liability insurance coverage for is a cost savings for your company***

Comprehensive In-Home Needs Assessment

  • Identify your loved one's special personal care needs
  • Evaluate daily living skills
  • Assess home safety and mobility needs
  • Assess mental health and screen for Alzheimer's/dementia
  • Address financial and legal issues (with 10 years of knowledge, we are familiar with reputable service providers)
  • Screen for eligibility for state-funded programs
  • Social and nutritional assessment

Immediate and Long-Term Care Plans

Arrangement and coordination of in-home services, which may include:
  • Delivered meals
  • Homemaker/companion services
  • Transportation
  • Selecting alternative living arrangements
Retirement and assisted living communities, and residential care homes have varying levels of care, costs, and social amenities.

Care Conference Family Advocate (ongoing or as needed):

You or a loved one is residing in a skilled nursing facility.  Care Conferences are many times missed by family due to distance or schedule conflicts.  A nurse from ASCS can represent your loved one and/or family in this situation.  A simple phone call to ASCS providing the basic background of client and concerns/questions the family has will be addressed.  Information regarding the conference will be provided to family via a professional report and sent US mail or email address provided.

  • Facilitate family meetings

Concerned about medical comprehension at doctor appointments for your loved one or yourself?

Provide the client’s previous Health History and Physical (H&P) from their primary health care physician and I can attend the appointment with the client.  The information can then be relayed to the family and additional questions/concerns will be answered.
